Level 1 - Basic Services
Protective supervision
1. Assistance with medications
2. Assistance with bathing, dressing, and grooming
3. Daily personal laundry services provided by facility
4. Assistance with minor incidental medical services
5. Provide transportation to medical and dental appointments
Level 2 - Includes Level 1 plus the following:
1. Increased supervision for confusion/memory loss
2. Increased supervision due to behavioral problems
3. Assistance with urinary day/night incontinence that includes full incontinent supplies (diapers, day pads, periwash, gloves, and any other supplies), toileting, and staffing time
4. Serving modified special diets
5. Assistance with major incidentals medical services
6. Assistance with loss of hearing w/o hearing aid
7. Paranoia and/or delusions and hallucinations
8. Blood sugar monitoring
9. Closely monitoring all eating habits
Level 3 - Includes Levels 1 and 2 plus the following:
1. Incontinence care of bladder/bowel. (Same as number 2 above)
2. Total supervision due to confusion, memory loss, and mobility
3. Supervision due to resident's wandering
4. Wheelchair dependency
5. Diabetes care
Hospice Care
Other services requiring a licensed person
1. Insulin/blood sugar for diabetes
2. Catheter care
3. Wound care
4. Etc.
